Pathway Plan
We will start to work with you to develop a Pathway Plan just before your 16th birthday.
The plan is there to make sure that you are as prepared as possible when you leave care.
Your plan will include information like:
- housing
- education or training
- job options
- money
- health and lifestyle
- support needs
As well as a Pathway Plan, you will have a Leaving Care Personal Advisor.
Their job is to:
- answer any questions or concerns that you have about leaving care
- review your plan with you every 6 months
- support you as you move to independent living
