You can continue to see the Looked After Children’s Nurse until you are 25 years old if you need support with health concerns.
Should you lose your Health Passport, you can get a replacement from the Looked After Children’s Nurse. If you don’t know how to contact the Looked after Children’s Nurse please speak with your Personal Advisor.
During your time in care, you will have been taken for regular visits to the dentist and doctor's appointments when you needed them.
